Policy:

ELITE HEALTH CARE SYSTEMS implement policies and procedures to accommodate patient privacy rights as required by and specified in the Privacy Rule of the Administrative Simplification provisions of the Health Insurance Portability and Accountability Act of 1996.

Procedure:

1) Patients cared for by this hospice have the following rights with respect to the privacy of their health information:
a) To receive a paper copy of the organization’s Notice of Privacy Practices
b) To lodge complaints about hospice’s privacy practices
c) To request restrictions on the uses and disclosures of health information d) To request to receive confidential communication
e) To access their protected health information for inspection and/or copying
f) To amend their health care information
g) To request anac counting of disclosures of health information

2) The privacy policies detail there requirements for each of the seright sand provide procedures for implementation

3) Staff and volunteers are oriented to ELITEHEALTH CARESYSTEMS’ policies and procedures and are there after provided with annual training regarding patient rights with respect to their health information.

4) Patients and families are informed of the ir rights with respect to the privacy of protected health information in the hospice’s Notice of Privacy Practices.
